This privacy policy covers Cash Clinical, Inc.’s treatment of personally identifiable information collected by Cash Clinical, Inc. through this website owned and operated by Cash Clinical, Inc.

This privacy policy does not apply to the practices of companies that Cash Clinical, Inc. does not own or control.

Collection and Use of Personal Information
You can visit the website of Cash Clinical, Inc. without revealing any personal information, however, Cash Clinical, Inc. needs certain personal information if you wish to contact us or be contacted by us for any reason.

By accessing the services of Cash Clinical, Inc. and voluntarily providing us with the requested personal information, you consent to the collection and use of the information in accordance with this privacy policy. Examples of this personal information include; email, name, and phone number.

Collection and Use of Non-Personal Information
Cash Clinical, Inc. automatically receives and records non-personal information on our server logs from your browser including your IP address and the pages you requested. Cash Clinical, Inc. may use this information to customize the advertising and content you see and to fulfill your requests for certain products and services. However, Cash Clinical, Inc. does not connect this non-personal data to any personal information collected from you.


Notice of Privacy Practices (HIPAA)

Personal Information:

Personal information is gathered by Cash Clinical Inc. for the sole purpose of providing lab testing to patients. Information is kept private from any outside source not affiliated with Cash Clinical Inc. Cash Clinical Inc. and those affiliated with Cash Clinical Inc. do not sell or give patient information to any third party without written permission from the patient.

Cash Clinical Inc. obtains payment at the time of service. Information concerning  insurance billing, private billing, and other related information is not collected.

Payment Privacy:

Payment information obtained regarding Credit Card, Personal Check, or Debit Card payments is kept confidential for use by Cash Clinical Inc. and Nova Credit Services. Receipts are held for two years by date in a secure location in cases of debatable transactions where receipts need to be obtained and delivered to Nova Credit Services. Cash Clinical does not handle disputes in cases of payment.

Waiting Area:

Cash Clinical Inc. is not responsible for patient privacy with regard to being noticed or seen by other individuals while waiting in the Cash Clinical Inc. waiting area.

Registration:

Registration for Cash Clinical Inc. is separate from the waiting area and provides a confidential location for registration. If the patient is accompanied by another individual, they must specify if that individual may be present during the registration process.

Drawing Area and Phlebotomist personnel:

All Cash Clinical Inc. personnel are required to obtain two unique identifiers (usually date of birth and phone number) for every patient. This redundancy can and will prevent potential errors.

The drawing area is a tight location and at times patient will over hear personal information being shared from Cash Clinical Inc. employees and patients. Employees are to limit the amount of personal information being exchanged and are to control their voice to speak softly. If pertinent information needs to be shared and there is no way of protecting so others do not over hear, the patient can be taken into a private area.

Results:

In accordance to Federal Law, patient reports are always reported to the licensed physician that ordered the testing provided. Other physicians may obtain a copy of the results by patient request either by written document or by hand written inclusion on the request slip from the patient.

Results will be faxed, mailed, or phoned to the physician’s office.
• Faxed results will include a Cash Clinical Inc. cover sheet in which a disclaimer will be written in cases of lost transmissions. • Mailed results will be folded so no patient information will face outward and placed inside a security envelope. • Phoned results will only be for panic values. If results are phoned to the physician’s office, a copy of the results will be mailed.

Patients may obtain their results from Cash Clinical Inc. during normal business hours at the front registration desk after completing the required paperwork. If there are patients currently waiting in the Cash Clinical Inc. waiting room prior to arrival the patient must wait their turn unless mutual agreement is made allowing for them to go ahead.
